If someone "initiates" a problem, and just leaves it there, they "said it first" is all. If someone initiates there is a problem and then opens the highway to rectify it, ergo an investigations, etc., then they not only addressed the issue, then provided a means to and end, so to speak. Identifying possible issues is great, but finding a way to rectify it leads to better governing in the future.
